1. Dr Lohse, in our performance-driven society, it has almost become normal to feel “exhausted”. In your view, at what point does exhaustion stop being a temporary low and become a signal from the body that requires conscious reflection on one's lifestyle?
Dr C. Lohse: That's right. Feeling drained and exhausted has become the norm for many of my patients. It's not surprising that tiredness and exhaustion are among the most common reasons people visit their GP these days.
Initially, tiredness is a completely normal signal from our body and isn't always a sign of illness. After intense periods of high stress or little sleep, it's natural for our bodies to need some rest. If we heed this signal and allow ourselves a period of rest, the symptom of fatigue usually disappears. Figuratively speaking, the battery is fully charged again.
However, you should be cautious if, despite taking a break, the tiredness persists for a longer period, or if you can no longer fully recharge your batteries and notice a significant drop in your ability to cope with everyday life. In that case, the exhaustion is manifest or pathological. With chronic fatigue, the battery is empty, figuratively speaking, and can only be recharged with a longer charging time.
In that case, it's worth taking a closer look at various lifestyle factors: how are your sleep quality, exercise, diet, stress management, and recovery times? Our body has many regulatory mechanisms, but it needs the right conditions to function properly. Persistent exhaustion can be a sign that your own balance is out of kilter.
In the extreme form, known as fatigue, recharging is no longer possible at all. In that case, a comprehensive medical assessment is required.
The limits of diagnostics: Why a blood count often tells only half the story
2. Many patients report that, despite normal standard laboratory values, they constantly feel at their limit in everyday life. Why is the “full blood count” often not enough in practice to identify the underlying causes of persistent fatigue and a lack of vitality?
Dr C. Lohse: While the classic blood count is an important part of basic medical diagnostics, it only provides a small snapshot. Many people automatically associate a “full blood count” with a comprehensive analysis of all relevant bodily functions. In reality, it primarily focuses on the blood volume and the various blood cells. Organ values such as liver and kidney values, inflammation markers, and metabolic parameters like the long-term sugar level HbA1c and the insulin resistance marker HOMA index are not included, but they are important parameters to assess in cases of persistent fatigue. In most cases, a micronutrient analysis is also advisable. To further assess an individual's energy metabolism disorder, determining the Bioenergetic Health Index (BHI) can be beneficial.
However, it's never a question of looking at individual laboratory values in isolation. Instead, the key is to understand how different organ systems interact and to interpret laboratory findings alongside the patient's medical history and a physical examination. Only this comprehensive view allows for a truly individual and holistic diagnosis, as well as targeted therapy based on it.
It's also important to know that some issues require special laboratory tests, which are generally not covered by statutory health insurance and are therefore usually offered as self-pay services. When interpreting these findings, it's also important to note that a value within the reference range doesn't automatically mean optimal supply or function. The functional assessment of micronutrients and metabolic processes, in particular, has so far played only a minor role in medical studies and specialist training. As a result, many patients find that their symptoms remain unexplained, even though their standard laboratory results are unremarkable. In such cases, a holistic or functional approach can help identify connections that go beyond traditional basic diagnostics.

Unlike traditional blood counts, which primarily measure cell numbers or organ values, the BHI assesses mitochondrial function directly within living blood cells (such as platelets or leukocytes). 3. Building on this quest for causes: In your view, is there a direct biochemical link between depleted energy levels and a corresponding increased susceptibility to external influences or infections?
Dr C. Lohse: Yes, there is a close biochemical link between cellular energy metabolism and the performance of our immune system. It's important to understand that every immune response is an energy-intensive process. As soon as pathogens are detected, immune cells must rapidly ramp up their energy production to multiply, produce messenger substances, and target pathogens. This energy is mainly supplied in the form of ATP by our cellular powerhouses, the mitochondria. At the same time, activated immune cells make targeted changes to their metabolism. This process is called immune metabolism. If the mitochondria aren't functioning optimally or if essential cofactors like magnesium, B vitamins, iron, copper, or other nutrients are lacking, energy production can't work as it should. This, in turn, can also impair the immune system's function.
Additionally, chronic sleep deprivation, ongoing psychological stress, and systemic inflammation simultaneously negatively impact both energy metabolism and the immune system. This means that exhaustion and increased susceptibility to infection often have the same causes – such as iron deficiency, thyroid dysfunction, chronic inflammation, or mitochondrial dysfunction.

Stress & nutrient consumption: Why reserves dwindle during periods of stress
4. Micronutrients play a central role in your practice. When we consider our modern lifestyle, why does our body often use up its vital substance reserves much faster during stressful periods than we notice in everyday life?
Dr C. Lohse: Our body is constantly adapting to external and internal demands. During periods of high stress, many biochemical processes run more intensively. To do this, the body needs various micronutrients as cofactors – little helpers for enzyme systems and metabolic pathways. This means that when we're under stress, our body also uses more micronutrients. At the same time, lifestyle factors like diet, sleep quality, or ongoing stress affect how well our bodies are supplied. That's why it's particularly worthwhile during challenging periods in life to consciously manage your own fundamentals, such as diet, exercise, and recovery. The problem, however, is that it's not that simple. Many of my patients find themselves in a vicious cycle. Because when we're stressed, we often turn to quick meals like fast food and ready meals. Firstly, we crave that combination of carbs and fat, and secondly, we have little time for healthy eating, cooking, or mindful eating. The problem is that this food is usually just empty calories. Yet, especially during stressful times, we need a particularly high amount of nutrients. And so the vicious cycle begins: a nutrient-poor diet and increased nutrient consumption exacerbate micronutrient deficiencies, making us even more sensitive to stress, vulnerable, and exhausted.
Magnesium as a foundation: support for nerves and mental resilience
5. In this context, magnesium often takes centre stage as an “all-rounder”. Why is this mineral such an important foundation when it comes to supporting both mental resilience and the normal functions of the nervous system?
Dr C. Lohse: Magnesium plays a role in over 300 metabolic processes in the body. It is particularly important for muscles, the nervous system, and energy metabolism. At the neurobiological level, magnesium acts as a natural regulator of the NMDA receptor, which is important for learning, memory, and processing stimuli. If there isn't enough magnesium available, this receptor can more easily become overactivated. This can lead to increased neuronal excitability, which may be accompanied by symptoms such as inner restlessness, heightened susceptibility to stress, or muscle tension. That's why I also like to call magnesium the “salt of inner calm”. Furthermore, magnesium influences the function of various neurotransmitter systems, including GABA (Explanation: We need to remove this, as there are no approved health claims for GABA), and indirectly affects serotonergic and dopaminergic signalling pathways. That's why I consider magnesium a key component of holistic therapy: it can support the normal functioning of the nervous system and promote physiological stress regulation.
Side fact: Magnesium as a natural NMDA receptor blocker The NMDA receptor (N-methyl-D-aspartate) is an ion channel in the brain that is activated by the excitatory neurotransmitter glutamate. Under physiological resting conditions, magnesium ions block the centre of this channel, preventing an uncontrolled influx of calcium into the nerve cell. If there is an extracellular magnesium deficiency, this regulatory blockade is removed. The receptor becomes hypersensitive to glutamate, which can lead to neuronal over-excitation (excitotoxicity), which is biologically associated with increased stress sensitivity. 6. Taking this idea further, magnesium is often referred to as the “spark plug” of cells. How does this mineral specifically help reduce daily fatigue and physiologically support normal energy metabolism?
Dr C. Lohse: The comparison to a “spark plug” is a nice image, because magnesium is involved in many processes where energy is provided and used in the body. Magnesium is essential for the formation of ATP and, therefore, for supplying energy to nerve cells. The brain, in particular, is one of the organs with the highest energy requirements. Our energy currency, ATP, is predominantly found in its biologically active form as a magnesium-ATP complex. Without magnesium, many energy-dependent processes wouldn't function optimally.

Immune function & micronutrients: Defence under high stress
7. Alongside energy, resilience is a major issue. How does a magnesium intake that meets our needs help our immune system maintain normal function, even under high stress, and thereby contribute to overall vitality?
Dr C. Lohse: Magnesium is essential for numerous processes within the immune system. As previously mentioned, virtually every immune response demands a high level of energy. Therefore, an adequate supply of magnesium is a crucial prerequisite for immune cells to maintain their function. Furthermore, magnesium also influences the activation and function of various immune cells, such as T lymphocytes, B lymphocytes, and natural killer cells (NK cells).
The need for magnesium often rises, especially during periods of physical or psychological stress. Stress hormones promote magnesium excretion via the kidneys, while simultaneously increasing energy expenditure and, consequently, the need for magnesium-dependent metabolic processes. If an existing deficiency isn't addressed, it can impair the normal functioning of the nervous, muscular, and immune systems.
However, the immune system is a highly complex network influenced by numerous factors. Alongside micronutrients, getting enough sleep, exercising, a balanced diet, and a strong microbiome also play a role.
Side fact: Magnesium and T-cell activation Magnesium's role in the immune system extends far beyond simply providing general energy. Molecular biology studies show that T lymphocytes (specific defence cells) have a surface protein called LFA-1, which is essential for recognising and combating infected cells. This protein can only adopt its active configuration when magnesium is bound to a specific binding site. Practical tip for resilience: A daily boost for greater vitality
8. Finally, if you could recommend a simple daily routine to our readers for greater resilience and vitality, what would be your top piece of advice from holistic practice?
Dr C. Lohse: My top piece of advice is surprisingly simple: once a week – on a Sunday, for example – take about 15–30 minutes to actively plan a healthy week. That's what I do too. I deliberately consider: What am I going to cook this week? When will I exercise? When do I take my supplements? When do I want to go to bed, and when do I want to get up? Write these goals down and schedule them with the same commitment as you would professional appointments. The reason for this is simple: health rarely comes from spontaneous decisions. When we're stressed, we usually revert to the habits that require the least energy – and these are often not the healthiest. By planning your week in advance, you reduce the number of daily decisions and increase the likelihood of actually sticking to healthy routines.
From my experience in practice, this half-hour of planning is often more effective than searching for the next “miracle cure”. Small habits, practised consistently week after week, have the greatest long-term impact on resilience, vitality, and health. Give it a try!
